Medical Release ( For Instruction Courses )
Note: Everyone participating in any instruction course will be required to complete a medical release. We HIGHLY suggest that you read over the medical release. If you need to answer yes to any of the questions, you will need to print out the medical release and have a doctor (familiar with diving) sign off that you are ok to dive, prior to starting your course.
